Questions about our online form

Urgent advice: Do not use the form in an emergency

Call 999 for a medical emergency. If you need urgent help and it is not an emergency, use 111 online or call 111.

About the form

What is happening to the way I contact the surgery?

You can use our new online form to request an appointment or get help with an admin question. The form asks you questions to help us understand what you need and get you to the right help, whether that is a GP, nurse or pharmacy appointment, without waiting on the phone at 8am. Your answers are sent to the team at Ariel Healthcare to review.

Why are you changing it?

The form helps us understand what you need before we speak to you. That means we can send you to the right person first time, and keep phone lines free for people who cannot use the form.

Do I have to use it?

No. You can phone us on 01460 67763 or come into the surgery. We will treat your request the same way.

What can I ask for?

You can use the form for health problems, like a new symptom, a long-term condition, or something you are worried about.

You can also use it for admin requests, like a sick note, a question about a repeat prescription, test results or a referral.

Can I use it if I am not registered here?

The form is for patients registered at Ariel Healthcare. If you are not registered with us, you can register with us, or get help from a pharmacy, 111.nhs.uk or 111 by phone.

What if it is an emergency?

Do not use the form. Call 999 for a medical emergency, or 111 if you need urgent help and it is not an emergency.

Using the form

Can I fill it in for someone else?

Yes, if you are their carer or parent. Requests for under-18s are always reviewed by a clinician rather than routed automatically.

Can I upload a photo (for example, a rash)?

Yes, you can attach a photo to help the clinician review your request. Please do not upload photos of sensitive areas.

How long does the form take?

Usually a few minutes. Questions adapt to what you tell us, so you are only asked what is relevant.

Do I need an NHS login or account?

You don't need to log in with your NHS account, but you can if you want to.

When can I use the form?

The form is open while the practice is open. When it is closed, it tells you when it opens again. If you need help before then, use 111 online or call 111. In an emergency, call 999.

I find forms difficult. What can I do?

Call us on 01460 67763 and we will fill it in with you. You can also ask someone you trust to help you.

After you send it

How long will it take to hear back?

We aim to get back to you by the end of the next working day. If your problem gets worse while you are waiting, call us or call 111.

Will I get an appointment?

Sometimes. The answers you give help us work out what kind of help you need. That might be an appointment with a GP, a nurse or another member of the team. It might be a phone call, or advice from a pharmacist, who can treat some conditions without you seeing a GP. We will tell you what we are offering and why.

What if I want to see a particular doctor?

Tell us in the form and we will try to book you with them. We cannot always do this, because it depends on their availability and how urgent your problem is. If you need help sooner, we may offer you someone else.

How will I get my appointment?

Someone at the practice will call, or you will receive a text with the details. The text will also contain a link to change or cancel the appointment. Some requests will come with a link to pick your own appointment at a time that suits you.

AI and safety

Does a computer decide what happens to me?

The form uses artificial intelligence (AI) to sort requests and work out how urgent they are. It does not decide your care on its own and it does not diagnose you. A member of the practice team looks at every request.

Is it safe?

Bookable Navigator is registered with the MHRA, the UK regulator for medical devices, as a Class I medical device. It meets the NHS clinical safety standard for health IT (DCB0129). Our clinical safety documents are available on request.

What if I do not want AI involved?

Phone us on 01460 67763 or come in instead.

Your information

What happens to the information I give?

Your answers go to Ariel Healthcare to be reviewed. Once your request is dealt with, a summary is filed to your GP record so your care team has the full picture.

The staff at Ariel Healthcare who are involved in your care. Healthtech-1 provides the form and stores your answers securely on our behalf.
