Long-term condition reviews

If you have a long-term condition, we will invite you for a review at least once a year. It is a chance to check how you are, make sure your treatment is still right for you and talk about anything that is worrying you.

We review conditions including:

  • asthma and COPD

  • diabetes

  • high blood pressure, heart disease, atrial fibrillation and stroke

  • chronic kidney disease

  • epilepsy and underactive thyroid

  • serious mental illness

  • learning disability

How it works

We invite you around your birthday month, by text message or letter. If you have more than one condition, we try to review them all at once.

  1. You may have a blood test, blood pressure check or other tests with a healthcare assistant first.

  2. A week or two later, you have your review with a nurse, pharmacist or GP, who will have your results.

What to bring

  • your medicines, inhalers or a list of what you take, including anything you buy yourself

  • home blood pressure or blood sugar readings, if you take them

  • we may ask you to bring a sample of your urine

  • any questions you want to ask

If you cannot come

Please let us know if you cannot attend, so we can offer the appointment to someone else and rebook you. You can send some things, like home blood pressure readings, through our online form.

Your yearly review is also when we review your medicines. If you miss it, we may only be able to issue short supplies of your repeat prescriptions until you have been seen.
